You will first want to include information for your care team denoting this attachment as erroneous.
- If available for your clinical team, you can locate the "Note" button in the Action Bar in the upper right of coach portal to input details as to which attachment is erroneous, including the date and time from the patient's timeline.
You can then submit a support ticket via support.welkinhealth.com and request that our team remove this attachment.
Lastly, you will want to remember to then upload the attachment to the correct patient profile.
