In this guide, we’ll walk through how to create a new Care Flow and how to edit an existing Care Flow in Welkin.
Patients can have multiple Care Flows. All Care Flows, even if completed, will remain at the top of a Patient’s Profile above their Timeline.
Important Note: Care Flows can be created automatically by a Process in Welkin or through an integration.
To manually create a new Care Flow in Coach Portal, follow the steps below:
- Navigate to the Patient’s Profile that you’d like to create a Care Flow for.
- In the Action Bar which is located in the upper right hand corner of the Patient’s Profile underneath the Search Bar you will find the Action “Care Flow”.
- Select Care Flow.
- A “Care Flow” pop-up will appear in the lower left corner of your screen.

Complete the applicable fields:
- The 1st tier Project (ex. “Care Flow”) field Title and field Description.
- You can add multiple 1st tier Projects by selecting “+ Add”.
- You can remove any 1st tier Projects by selecting “Remove”.
- The 2nd tier Task (ex. “Goals”) field Title and field Description.
- You can add multiple 2nd tier Tasks by selecting “+ Add”.
- You can remove any 2nd tier Tasks by selecting “Remove”.
- The 3rd tier Sub task (ex. “Intervention”) field Title, field Assignee of the Sub task, and field Reminder Date.
- You can “assign” a Sub task to yourself or to another Coach.
- By selecting a “Reminder Date”, an Alert will be sent to the Inbox of the Assignee when the Sub task is due.
- You can add multiple 3rd tier Sub tasks by selecting “+ Add”.
- You can remove any 3rd tier Sub tasks by selecting .
- To create the new Care Flow, select Save Care Flow.
To edit an existing Care Flow in Coach Portal, follow the steps below:

Navigate to the Patient’s Profile of the Care Flow that you’d like to edit.
- Select the Care Flow from the top of the Patient’s Profile above their Timeline.

Select the pencil icon.
- A “Care Flow” pop-up will appear in the lower left corner of your screen populated with the Care Flow you selected.
- You can edit, add, and/or remove any of the fields.
- To save your changes, select Save Care Flow.
Important Note: If Care Flows, or fields within Care Flows, are deleted that information that was removed will not be in the Patient’s Profile or Timeline.
